Intestinal malrotation is a congenital anomaly of rotation of the midgut. It occurs during the first trimester as the fetal gut undergoes a complex series of growth and development. Malrotation can lead to a dangerous complication called volvulus, in which cases emergency surgery is indicated. Malrotation can refer to a spectrum of abnormal intestinal positioning, often including:
The small intestine found predominantly on the right side of the abdomen
The cecum displaced from its usual position in the right lower quadrant into the epigastrium or right hypochondrium
An absent or displaced ligament of Treitz
Fibrous peritoneal bands called bands of Ladd running across the vertical portion of the duodenum
An unusually narrow, stalk like mesentery
The position of the intestines, narrow mesentery and Ladd's bands can contribute to several severe gastrointestinal conditions. The narrow mesentery predisposes some cases of malrotation to midgut volvulus, a twisting of the entire small bowel that can obstruct the mesenteric blood vessels leading to intestinal ischemia, necrosis, and death if not promptly treated. The fibrous Ladd's bands can constrict the duodenum, leading to intestinal obstruction.

Признаки и симптомы
Признаки и симптомы мальротации варьируются в зависимости от возраста и от того, страдает ли пациент от острого заворот кишки или испытывает хронические симптомы. Если у пациента, чаще всего новорожденного, остро проявляется заворот средней кишки, это обычно проявляется желчной рвотой, схваткообразными болями в животе, эпизодическими желудочно-кишечными кровотечениями, вздутием живота, а в поздних случаях – выделением крови и слизи со стулом. Поздняя (постнатальная) манифестация характеризуется нечеткими симптомами, которые могут включать боли в животе, напоминающие мигрень, повторную рвоту, нарушение роста и развития, потерю веса и другие неспецифические проблемы с желудочно-кишечным трактом.
Причины
Точная причина кишечной мальротации неизвестна. Она не связана с каким-либо конкретным геном, однако имеются некоторые данные о повторных случаях заболевания в семьях.
Диагноз
Мальротация чаще всего диагностируется в младенчестве, однако некоторые случаи выявляются лишь в более позднем детстве или даже во взрослом возрасте. Рентгенологическое исследование верхних отделов желудочно-кишечного тракта с барием является методом выбора для оценки мальротации, поскольку оно часто показывает аномальное положение двенадцатиперстной кишки и дуодено-йеюнального изгиба (связки Трейтца). В случаях мальротации, осложненной заворотом кишки, рентгенологическое исследование верхних отделов ЖКТ демонстрирует картину "пробки" дистальной двенадцатиперстной кишки и тощей кишки. В случаях обструкции лентами Лэдда рентгенологическое исследование верхних отделов ЖКТ может выявить обструкцию двенадцатиперстной кишки. Хотя рентгенологическое исследование верхних отделов ЖКТ считается наиболее надежным диагностическим тестом при мальротации кишечника, ложноотрицательные результаты могут наблюдаться в 5% случаев.
